Job Description:

A company is looking for a Bilingual Spanish Telehealth RN Case Manager, SNP with a California RN License.

Key Responsibilities:

Coordinate care for patients, families, and physicians
Assess and develop care plans for members with complex and chronic care needs
Monitor and evaluate the effectiveness of care management plans and modify as necessary

Required Qualifications:

Minimum of 1 year clinical experience
Minimum of 3 years case management experience
Active, valid, and unrestricted RN license in California
Health plan experience preferred
Case management certification preferred
